THE PURPOSE OF THIS STUDY
The purpose of this study is to explore how occupational therapists working with individuals at the EOL are currently helping clients increase their sense of meaning-in-life (MIL), elevate what else can be done to enhance clients' sense of MIL, and identify the typical barriers encountered by occupational therapists when assisting with enriching MIL in EOL care.

Although the results of this study may be published, no information that could identify you wi.l be included. Your responses will be coded and kept in a password protected computer.
Your participation in this study is voluntary. If you choose to participate, you may quit the survey at any time without negative consequences. You can also choose not to answer any survey questions that you do not wish to answer. No service to which you are otherwise entitled will be lost or jeopardized if you choose not to participate in the study or quit partway through the study.

If you agree to take the survey, it is implied that you have read the information above about the research, your rights as a participant, and give your voluntary consent.
Dear Prospective Research Participant:
We are graduate students from San Jose State University, asking for your help and cooperation in a collaborative research project investigating the effects of occupational therapy on finding meaning in end-of-life care. The research is important in understanding how occupational therapy can provide a valuable service to this population.
You are invited to participate in and/or pass along this study to qualified colleagues. Data is being collected from English speaking occupational therapists ages 25-70, with a minimum of three years of experience working in end-of-life care, who are currently working in end-of-life care or who have worked in end-of-life care in the past ten years. There are no direct benefits to you for participating, however, the data will be useful in further expanding the knowledge of occupational therapy’s value working with this population.
The project involves a survey consisting of demographic questions and then questions that include a series of hypothetical situations that may occur in end-of-life care. We are interested in your response in each scenario. Participation is expected to take approximately 20 minutes.
Your participation is entirely voluntary. If you agree to participate, you may choose not to answer any given questions, and you may withdraw your consent and discontinue your participation at any time. All of your answers will be recorded anonymously.
